I, the undersigned (releasor), have chosen to purchase eyelash extensions from INFINITY LASH STUDIO, LLC. By participating in the eyelash extension procedure and for other valuable considerations, I agree to this waiver of liability, general release, and covenant not to sue (this “release”), and agree as follows.

I authorize INFINITY LASH STUDIO, LLC to perform the semi-permanent eyelash procedure. I understand this involves attaching individual synthetic eyelashes to my natural eyelashes using adhesives. I agree to keep my eyes closed and remain still during the procedure, and follow all instructions from the lash artist and other INFINITY employees, both during and after the procedure. I have been fully informed about the method, procedures, and potential risks, including redness, irritation, and allergic reactions. I am not influenced by any statements made by INFINITY LASH STUDIO, LLC employees.

I consent to this procedure at my own risk. (INITIALS)

I confirm that my responses on the INFINITY LASH STUDIO, LLC client form are complete and accurate, including any allergies and medical conditions that might affect this procedure.

If I feel uncomfortable at any time during the eyelash extension procedure, I will inform the lash artist and can stop the service at any time. (INITIALS)

I agree, on behalf of myself and my heirs, representatives, assigns, and insurers, to release and hold harmless INFINITY LASH STUDIO, LLC and their members, employees, and agents from any and all claims or causes of action related to the lash extension procedure.

This includes claims of any kind, whether known or unknown, related to my participation in the procedure, any negligence by INFINITY LASH STUDIO, LLC or any third party, and the results of the procedure. (INITIALS)

I agree, and also agree on behalf of my affiliates, not to file any claim, lawsuit, or other proceeding against INFINITY LASH STUDIO, LLC and its members, employees, and agents for any reason. I will indemnify, defend, and hold them harmless against any claims or liabilities arising from any lawsuit, claim, or proceeding filed by me or my affiliates.

INFINITY LASH STUDIO, LLC and its members, employees, and agents do not need to seek recovery from any insurer or third party to maintain a claim against me. (INITIALS)

APPOINTMENT CANCELLATION POLICY:

To be respectful of the needs of other clients and to avoid cancellation fees, we require a minimum of 48 hours' notice for any cancellations, reschedules, or appointment adjustments. If you cancel or reschedule less than 48 hours before your scheduled appointment time, you will be charged 50% of the service fee. No-shows and cancellations without prior notice will be charged 100% of the service fee. I am aware that this is a non-refundable service. I understand and agree to the policy.

I authorize Infinity Lash to confirm bookings over the phone without my presence. I acknowledge that my card will only be charged in accordance with the terms and conditions agreed upon with Infinity Lash. By signing, I consent to my information being saved on file for future transactions. This includes, but is not limited to, deposits, membership purchases billed in a timely manner, or any transactions for goods or services on a prearranged, recurring billing schedule. There is a chance you could have an allergic reaction to the primer, glue, or other products used during the lash extension service. We offer a patch test to check for allergies. If you choose not to do the patch test, you accept the risk of any allergic reactions during the lash extension service.
